Job Summary
Providing nursing care remotely to a defined group of patients, the full-time Washington State Licensed RN Nurse will assess patient needs, develop care plans, and collaborate with the healthcare team to ensure effective health service delivery.
Key responsibilities:
- Conduct ongoing assessments and develop individualized care plans based on patient needs
- Provide telephone consultations and education to patients and their families regarding health care goals and interventions

Required qualifications:
- Associates degree in Nursing from an accredited program
- Registered Nurse License in Washington or a Compact License at hire
