West Virginia Licensed Social Worker
Job Summary
To support patient-centered care, the full-time remote West Virginia Licensed Social Worker will provide social work services including counseling, crisis intervention, and group leadership to patients and their families while collaborating with interdisciplinary teams.
Key responsibilities
- Conduct psychosocial assessments to identify emotional, social, and community strengths and challenges related to patients' diagnoses and life situations
- Develop and implement comprehensive treatment plans utilizing evidence-based interventions while participating in multi-disciplinary health care teams
- Provide education and support to patients and families regarding adaptation to diagnoses, treatment, and available community resources
Required qualifications
- Master's degree in social work or a related field
- Current West Virginia social worker licensure (LGSW, LCSW, or LICSW) at the time of hire
- Ability to obtain master's level social work licensure in Maryland within six months of hire
- Two years of social work experience
- Certification in Case Management is preferred
