Job Summary
Providing social work services to patients and families, the full-time remote West Virginia Licensed Social Worker will conduct psychosocial assessments, develop treatment plans, and connect individuals with community resources to address their health-related social needs.
Key responsibilities
- Conduct psychosocial assessments to identify emotional and social strengths and challenges related to patients' diagnoses and life situations
- Collaborate as a member of an interdisciplinary treatment team to formulate and implement comprehensive treatment plans
- Facilitate individual and group therapy sessions, providing crisis intervention and education to patients and families
Required qualifications
- Master's degree in social work or a related field
- Current West Virginia social work licensure (LGSW, LCSW, or LICSW)
- Ability to obtain master's level social work licensure in Ohio within six months of hire
- Two years of social work experience
- Certification in Case Management (preferred)
