Wisconsin Licensed Care Manager
Job Summary
Supporting post-discharge members, the full-time remote Wisconsin Licensed Care Manager will assess needs, coordinate care services, and facilitate transitions for Medicaid members, primarily focusing on behavioral health treatment and support.
Key responsibilities
- Evaluates member needs through post-discharge assessments and medication reconciliation
- Develops care/service plans in collaboration with interdisciplinary teams to support member transitions
- Facilitates education and resources to enhance member wellness and overall quality of care
Required qualifications
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
- Active clinical license in Wisconsin (LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN)
- 2-4 years of related experience in case management or behavioral health
- Strong knowledge of community resources and support services
- Experience with post-discharge care coordination
