Job Summary
Supporting the Start Smart for Baby program, the full-time remote Wisconsin Licensed Care Manager (RN) will develop and facilitate personalized care plans, assess member needs, and coordinate care management activities to ensure high-quality healthcare outcomes.
Key responsibilities
- Evaluate member needs and barriers to care, recommending plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address unmet needs
- Provide education and resource support to members and their families on healthcare options and services
Required qualifications
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in OB, NICU, and postpartum care
- Active RN license in Wisconsin or a Compact State Licensure
