Job Summary

To support staffing needs, the part-time Wisconsin Licensed Case Manager RN will manage patient assessments, coordinate care, and ensure medical necessity while working remotely with flexible hours, including potential weekend shifts.

Key responsibilities

• Manage patient assessments and care coordination to ensure appropriate levels of care
• Utilize clinical expertise and critical thinking skills to determine medical necessity
• Collaborate with a multidisciplinary team to enhance patient outcomes and experiences

Required qualifications

• Bachelor's Degree in Nursing
• Licensed as a Registered Nurse (RN) in the State of Wisconsin or under the nurse licensure compact
• Three (3) years of relevant clinical nursing experience
• Recent experience as a hospital-based Nurse Case Manager is preferred
• Master's Degree in Nursing or a healthcare-related field is preferred
