Wound Care Nurse Practitioner

Job Summary

To support a growing home health program, the remote Wound Care Certified Nurse Practitioner will manage care for members with complex and chronic wounds, develop individualized treatment plans, and utilize technology to assess and coordinate care.

Key responsibilities

• Manage wound care through closure by developing and reassessing individualized treatment plans
• Assess wounds using clinical data and technology, independently determining diagnosis and treatment
• Recognize complications and coordinate timely evaluations by relevant specialists

Required qualifications

• Active, unrestricted NP license with the ability to obtain licensure in applicable states and current national NP board certification
• Wound care certification or advanced specialty training in wound management
• 3-5 years of clinical experience managing chronic and complex wounds
• Strong clinical assessment and decision-making skills
• Experience with coding and clinical documentation practices
