Pennsylvania Licensed Case Manager
Job Summary
A company is looking for a Case Manager to guide and assist individuals in accessing necessary medical, rehabilitative, and social services.
Key Responsibilities
- Assigns and schedules Behavioral Health Technician (BHT) workers to maximize service coverage
- Monitors productivity and utilization of assigned units, adjusting assignments as needed
- Coordinates Individualized Service Plans (ISP) team meetings and maintains accurate client files
Required Qualifications
- High School Diploma or equivalent with two years of direct experience working with children and/or families
- Bachelor's Degree preferred with two years of direct experience; Master's Degree with one year of experience is also acceptable
- Previous case management experience is preferred
- Must have Act 33 and 34 clearances, including FBI clearances
