A senior healthcare company is in need of a Remote Bilingual Spanish Registered Specialized Nurse Practitioner Transition of Care Case Manager.

Engage with our newly discharged SNP members
Conduct holistic assessments
Develop individualized care plans

Required Skills:

A minimum of 4 years of RN experience in an acute setting
An unrestricted RN license obtained in the United States with the ability to secure other licensure states
Telephonic experience
Bilingual - Spanish
