Remote Case Management Analyst
Job Summary
A company is looking for an Analyst, Case Management Specialist.
Key Responsibilities
- Interact with low stratification members via phone, coordinating care and updating assessments and care plans
- Create care plans based on assessments and claims data for members to follow
- Participate in Interdisciplinary Team meetings to discuss members' healthcare needs and barriers to care
Required Qualifications
- 3+ years of experience in a health-related field
- 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook)
- Bachelor's Degree or equivalent experience required
- Familiarity with community resources and services
- Strong organizational skills and ability to work independently
