Remote LPN Case Manager
Job Summary
A company is looking for a Case Manager LPN (Licensed Practical Nurse).
Key Responsibilities:
- Maintain remote access to required EHR systems
- Conduct data mining to close quality gaps within EHR systems
- Contact provider offices to request medical records
Minimum Qualifications:
- Active Licensed Practical/Vocational Nurse (LPN/LVN) license
- At least 1 year of experience in provider outreach and/or EHR system navigation
- At least 1 year of experience with HEDIS /STARs data abstraction
- At least 2 years of experience in data analysis and quality chart reviews
- Proficiency in Microsoft Office applications, including Word and Excel
