RN Case Manager
Job Summary
A company is looking for a RN Case Manager.
Key Responsibilities:
- Assess, plan, implement, and coordinate all case management activities with members to evaluate their medical needs and facilitate overall wellness
- Collaboratively develop proactive courses of action to address issues and enhance short and long-term outcomes
- Utilize case management processes to reduce risk factors, address complex health issues, and integrate available benefits
Required Qualifications:
- Active and unrestricted RN license in state of residence
- 3+ years of acute care clinical experience as an RN
- Ability to use a computer station with multiple screens and operate multiple programs simultaneously
- Private designated workspace free of distractions and high-speed internet
- Associates Degree in Nursing required, BSN preferred
