Job Summary
A company is looking for a Community Health Worker.
Key Responsibilities
- Provide culturally appropriate complex care management in a primary care setting
- Assist patients in overcoming barriers and navigating the healthcare system
- Support patients in making meaningful progress toward health-related goals over a 6-month period
Required Qualifications, Training, and Education
- High school diploma or GED preferred
- Valid driver's license and clean driving record if driving is required
- Two years of experience in a social service or healthcare setting preferred
- Must be a resident of the community served with knowledge of local resources
- Basic computer skills preferred
