Job Summary
A company is looking for a Proactive Titration LPN.
Key Responsibilities
- Contact patients to review medication titration recommendations and ensure adherence to treatment plans
- Maintain detailed documentation of patient interactions in compliance with HIPAA and clinical protocols
- Provide empathetic communication with patients and healthcare providers while managing responsibilities independently
Required Qualifications
- Active LPN license in a Compact Nursing State
- 5+ years of clinical experience in acute or ambulatory care, preferably in cardiology or nephrology
- Experience maintaining HIPAA-compliant environments
- Must have a HIPAA-compliant home office setup with reliable internet
- Strong track record of working independently and managing time effectively
