Job Summary

A company is looking for a Behavioral Health Case Manager.

Key Responsibilities
  • Coordinate and support mental health care for high acuity patients and their families
  • Develop, monitor, and evaluate treatment plans while facilitating transitions to community-based services
  • Collaborate with a multidisciplinary team to ensure efficient patient care and enhance satisfaction
Required Qualifications
  • Experience in clinical case management, particularly with high acuity patients
  • Understanding of mental health needs and social services in the community
  • Ability to assess crisis situations and respond appropriately
  • Certification in CANS/ANSA or willingness to obtain
  • Adherence to HIPAA regulations and professional ethics
