Job Summary
A company is looking for an LVN Case Manager - Disease Management.
Key Responsibilities
- Conducts clinical evaluations and assessments to determine case management eligibility and individual needs
- Develops and implements case management plans in collaboration with members, caregivers, and healthcare professionals
- Maintains a caseload for regular outreach, monitoring, and documentation of interventions and goal achievements
Required Qualifications
- Graduation from an accredited Licensed Vocational Nurse program
- Current unrestricted LVN license in Oregon
- 2+ years of recent clinical experience as an LVN/LPN
- 1+ years of experience in care management, disease management, or discharge planning
