Job Summary
A company is looking for a Case Manager RN - Work From Home - PST.
Key Responsibilities
- Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
- Develop proactive strategies to enhance members' short and long-term health outcomes
- Utilize clinical tools to evaluate member needs and refer to appropriate programs
Required Qualifications
- Active and unrestricted RN license in state of residence, preferably compact
- Willingness to obtain additional state licenses upon hire
- 3+ years of acute care clinical experience as an RN
- Associates in nursing or BSN
