Job Summary
A company is looking for a Field RN Case Manager in the Toledo Region, OH.
Key Responsibilities
- Manage and coordinate patient care in the home health setting
- Conduct assessments and develop care plans for patients
- Collaborate with healthcare providers to ensure quality care delivery
Required Qualifications, Training, and Education
- RN Associate's Degree or higher
- Current unrestricted state licensure as an RN
- Case Management Certification (CCM) preferred
- Minimum of 3 years of clinical experience
- Experience in home health preferred
